How much does short-term disability coverage cost?

Am I eligible?
You must be a US citizen or Green Card holder, actively at work with a minimum earnings based on your state of residence. Certain medical preconditions may make you ineligible, for example, if you have suffered a stroke, heart attack or had cancer in the last five years.
